National Cancer Institute Groin Expect Setbacks Sandgate “Efficacy” is how well a treatment works in ideal circumstances, such as in a carefully contrived scientic test. Unfortunately, real life is rarely ideal! (You may have noticed.) “Effectiveness” is how well the same thing works in typical clinical settings and patients’ lives. Which is what matters to most patients. A 2015 editorial in the British Journal of Sports Medicine explains that exercise (in a physical therapy context) is well-known to be efficacious, but often is not effective. That is, it works well when tested in the lab, but not so much for real patients. Again, effectiveness is what matters to patients! If effectiveness is low, only a few lucky and/or disciplined people can realistically expect to benefit. BACK TO TEXT
The cause is almost always related to volume and/or load: too much or too little. Acute spikes up and down (yes, increases AND decreases) in weekly volume have a high correlation to injury risk in athletes. When looking at the Acute : Chronic workload ratio, we look at the total volume performed by the athlete over the past Week, and divide that by the total volume over the past 4 weeks. What we would like to see is a ratio between 0.8 – 1.3. That means this past week’s volume has either been slightly decreased (0.8) or increased (1.3) compared to what the athlete is physiologically adapted to. If that ratio comes out to be a 1.5 or higher, this is an acute spike in training volume that puts them at a much higher risk of injury2.

Tendinopathies are one of the most common sports/musculoskeletal injury in modern western societies. The most common tendinopathies of the upper limb are Rotator cuff (mainly supraspinatus) and lateral elbow tendinopathy (LET) usually seen in sports such as volley - ball, tennis, basketball, swimming and so on whereas Achilles and patellar tendinopathy are the most common tendinopathies of the lower limb commonly seen in sports such as volley-ball, soccer, running, jumping and so on. Many physical therapy strategies have been proposed for the rehabilitation of tendon disorders. These strategies are electrotherapeutic such as therapeutic ultrasound, extracorporeal shockwave therapy, low level laser, iontophoresis and non-electrotherapeutic modalities such as eccentric training, soft tissue techniques, taping and needle therapy (Table (Table1).1). These treatments intend to reduce pain and improve function in tendinopathy but act in a totally different mechanism of action. Generally speaking, the efficacy of a treatment is based on reversing the pathology of the tendinopathy and not only improving the symptoms. Nowadays, eccentric exercise program is the most effective conservative approach in the treatment of tendinopathy[2,3].

These stages progressively move into one another, so there’s no time where you may only be in one stage and then immediately go to another stage. It’s most likely that you will be between stages of normal and reactive tendinpathy or between stages of reactive tendinopathy and tendon dysrepair. A trained medical professional may be able to give a diagnostic analysis of where you likely are according to the presentation of signs and symptoms and reaction to training.

You probably like exercise for its quiet, head-clearing benefits. Matthews recommends activities like endurance sports (swimming, cycling, running), one-on-one personal training, rock climbing, golf, or mind-body disciplines like yoga and Pilates. “Mind-body exercise is non-judgmental in nature and is more process-oriented versus goal- or performance-oriented,” she says. “This allows introverts to feel comfortable.”

The Question: ZIP Code: Shoulder impingement can weaken the rotator cuff, so physical therapy is key to alleviating this pain and preventing further damage. "Since one of the main jobs of the rotator cuff is to stabilize the joint, any weakness to it will lead to further impingement, creating an ever-vicious cycle," says Dr. Stark. Properly strengthening these muscles and balancing all the muscles and tissues that affect the position of the shoulder effectively can relieve the impingement.

This is especially true since the body cannot make all of the needs of collagen production itself. Vitamin C enriched gelatin improves collagen production. Take pre- and post-exercise.
